What do I do about Xbox 360 message "This disc is unreadable"?

it started when i was playing Call of Duty 4 (live) on some maps it would appear and bring me to the Xbox main menu then later those same maps would work fine. I got Call of Duty WaW and it happens again with a few maps but its only on Xbox live?
I considered the disk so i clean and "repaired" them both with a special kit. Is it the system and can i fix it?
